Please Note the Following Regarding Required Submission of District SA-132 (Bonds & Refunding Bonds), SA-135 (BANS) borrowing forms and SA-139 (Request for Building Project Data) building aid forms to the State Aid Office

Effective immediately, please send completed SA-132, SA-135 & SA -139 District building forms as noted above via email to Buildingforms@nysed.gov, including your district name & the relevant form submission type (SA-132, SA -135 or SA-139) in the subject line.

NOTE: Do NOT email Final Cost Report (FCR) forms to the State Aid Office.
